Do you look in the mirror and see tired eyes, even when you have slept well?

For many people, the earliest signs of aging are around the eyes. Excess skin may start to cover the upper lids. Bulging fat and wrinkles may appear in the lower lids.

Blepharoplasty (eyelid lift surgery) is one of the most commonly done cosmetic surgery operations. But our eyes are vital to our sense of who we are. Surgery must lead to you looking like yourself, only better.

Your eyes may make you look tired or fatigued due to several factors. Much of your appearance is handed down from your parents. When I see bulging fat in the lower lids I usually ask whether the patient looks like her father or mother and the answer is usually a definite link to one or the other.

The upper eyelids may develop considerable redundant skin. Skin may actually hang completely over the eyelids and rest on the eyelashes. In extreme circumstances, it may cause some to peripheral vision obstruction. When that occurs, medical insurance may cover part or all of the cost.

If the eyes were a painting the eyelids would be the frame around it. Sagging and redundant skin, bulging and excessive fat can cause the eyes to look old, sad and tired. Sometimes the very muscle that opens the eye is weak or stretched. Blepharoplasty or eyelid lifts may involve:

  • Local, local with sedation (twilight) or general anesthesia
  • Removal of excess skin
  • Shifting augmenting or removal of fat
  • (occasionally) tightening or shortening the muscle which opens the eye (levator)
  • Combining with facelift, brow/forehead lift or other procedures

Endoscopic Browlift and Revision Facelift

For years, caricaturists have understood the link between the emotions and their expressions. In the best of animation, emotion is conveyed by simple but easy-to-understand lines.

Much of emotional expression relates to position and shape of the brows.

Some of this may be helped by the use of Botox, which works by paralyzing the frown muscles and preventing them from pulling the brows down and into a chronic frown. It is a little like the treatment of heart disease: Dr. Gelfant understands that some of his patients are well treated with medicines and others are better off treated by surgery. With the brow, some patients may be well treated with botulin injections every 4-6 months whereas others are better off, especially if they are having eyelid surgery or a facelift done simultaneously, to have a brow lift at the same visit.
